Website privacy policy

How information submitted through this website is handled.

This page covers the website only. It is not the practice’s HIPAA Notice of Privacy Practices, which describes how your protected health information is used and disclosed in the course of treatment. That notice is provided separately at your first appointment.

What this website collects

This site is a static informational website. It does not create user accounts, does not sell products, and does not use advertising trackers or third-party marketing pixels.

The only information collected is what you voluntarily submit through the consultation request form: your name, email address, phone number, whether you are inquiring for yourself or a family member, your preferred time to be contacted, how you heard about the practice, and any logistical notes you choose to add.

Why the form deliberately does not ask about your health

The consultation request form is designed not to collect clinical information. You are explicitly asked not to include symptoms, diagnoses, medications, or treatment history. This is intentional: limiting what is transmitted through a web form is the most reliable way to protect it. All clinical discussion happens by telephone or in a scheduled appointment.

If you include health information in the form anyway, it is treated as confidential and handled with the same care as any other patient information — but please don’t.

Where form submissions go

Submissions are transmitted over an encrypted connection and delivered into a Google Workspace account operated by the practice under a Business Associate Agreement with Google. Access is limited to Dr. Hariri and authorized office staff. Submissions are retained only as long as needed to respond to your inquiry and, if you become a patient, are handled thereafter under the practice’s HIPAA policies.

Analytics and third-party services

This website may use a privacy-oriented, cookie-free analytics service to count page views and understand which pages are useful. Any such analytics is configured to exclude pages where information is submitted, and it does not attempt to identify individual visitors. This site does not use Google Analytics, Meta/Facebook pixels, or advertising retargeting tags on pages that collect information.

Web fonts are loaded from Google Fonts, which means your browser makes a request to Google’s servers and Google receives your IP address as part of that request. No other third-party content is embedded.

The site is hosted on Cloudflare, which processes standard server logs, including IP addresses, for security and reliability purposes.

Cookies

This website does not set advertising or tracking cookies. Your hosting provider may set strictly necessary cookies for security purposes.

Email and text messages

Ordinary email and SMS are not fully secure. Please limit email and text communication with the office to scheduling and logistics. Do not send clinical information, images, or documents containing health details by unencrypted email.

Children

This website is directed to adults. The consultation form is intended to be completed by a parent or legal guardian when the prospective patient is a minor.

Your choices

You may request that the practice delete an inquiry you submitted through this website by contacting the office. California residents have additional rights regarding personal information under California law; contact the office to exercise them.

Changes to this policy

This policy may be updated as the website changes. The date at the top reflects the most recent revision.
